What is a primary health need?
The key eligibility idea behind NHS Continuing Healthcare, explained without jargon.
Updated June 2026. A primary health need is the central eligibility idea in NHS Continuing Healthcare. In simple terms, it asks whether a person’s overall care needs are primarily health needs rather than needs that can lawfully be met by ordinary social care.
Quick answer: A primary health need means the overall care needs are mainly health needs rather than needs that can be met through ordinary social care.
This can be one of the most confusing parts of the CHC process because it is not based on one diagnosis, one incident, or one single care task.
What does primary health need mean?
The assessment looks at the total picture of a person’s needs. NHS guidance considers the nature, intensity, complexity and unpredictability of those needs.
That means the question is not simply, “Does this person need help?” Many people need help with daily life. The CHC question is whether the overall level and type of need goes beyond what social care is expected to provide.
Why diagnosis is not enough
A diagnosis such as dementia, Parkinson’s disease, stroke or another long-term condition may be very important, but it does not automatically decide eligibility.
Two people with the same diagnosis can have very different risks, levels of supervision, nursing input, behaviour, mobility, medication or nutrition needs. This is why the assessment focuses on needs rather than labels.
How does this appear in the assessment?
The Decision Support Tool records needs across care domains and then considers whether those needs show a primary health need overall. You can read more in our article on the Decision Support Tool.
What families should take from this
If you only remember one thing, remember this: NHS Continuing Healthcare is not decided by savings, care home fees or diagnosis alone. It is about assessed health and care needs.

Process lesson
The primary health need question is where separate facts come together. Assessment paperwork usually matters most when it shows the nature, intensity, complexity and unpredictability of needs in a clear way.
